I'm using the default theme and no extensions for both.

and I don't notice any problems with my toolbar.
The 2003-01-29 build works fine for me, but the 2003-02-01 build doesn't. Like Jeff, I get weird toolbar issues once I've customised the toolbar. For instance, if I add 'print' and 'new tab' buttons and the throbber, and restart Phoenix, some newly-added buttons won't show up anymore. If I restart again, even more buttons are missing. Very weird, indeed.

i have the same problem
with the windows 20030202 nightly....only happens after i customize the toolbar.so for me to use the browser...i cant change none of the buttons in the toolbar.

have you tried clicking 'Get New Themes' ?
it may have a text selection cursor ...
I just submitted a bug report on Bugzilla for this, but I filed it as "Trivial" since no functionality is lost.
