What happened to the form manager? If it was removed, I will have to go back to Seamonkey 1.1.18

It wasn't removed, it was replaced by the toolkit version that Firefox has (a lot of items were "inherited" from that toolkit and in part need to be extended to fit the needs for SeaMonkey again, but that step was necessary to avoid dealing with the ancient XPFE code any longer).
The major difference is that the new version keeps a history of form items entered on a per-field basis, thus not specific to a website. For example, if a form has a field "address" and you enter some value, it is memorized and made available next time to visit that page - but also any other page which happens to have an item called "address" too. Thus, that's one of the pitfalls with the new manager that you may get on different sites the same suggestions as for another site as they are sharing the same field identifier. A couple of days ago an enhancement request popped up, asking to make the old form manager available again. This implies that someone will have to step up, inspect the old code, and then reimplement it within the framework of the toolkit. Note that the report also points to an extension for a more user-friendly interface to inspect and edit currently stored form data. There was talk today (I believe) of an existing FF form manager extension being ported shortly to SeaMonkey?

I'd been pondering whether filing a bug on bringing back a dialog for the password manager would just be stomped on, but if this one has got through then maybe it's worth a shot.

Apparently Form History Control has been SeaMonkey 2.0 compatible since SeaMonkey 2.0a3. Phil
