Tools > Options > Privacy, select "Use custom settings
for history", click on Show Cookies and Remove All. The cookies
are gone but under "Site" there are many folders that don't get
removed.
After clicking 'Remove All', while still in the dialog, the 'Sites' (or at least some) still do display, bu there are no actual cookies in the "folders" therein?

(Isn't there a way in FF to tell it to exclude certain cookies from deletion?)
I booted in safe mode
The computer (rather then FF)? Shouldn't be necessary, but OK.
deleted the sqlite and the sqlite.bak
This is with FF close, correct?
when I start Firefox cookies.sqlite is back

It would be automatically recreated, though should be empty.

That is a possibility.
cookies.sqlite-shm and cookies.sqlite-wal
Generally should not be visible after FF has been closed.
Fire 750, bring back 250.

As a test, create a new, clean Profile.
Open it up, check the cookies there.
(I suppose some "mozilla" related cookies may show up.)
Remove all cookies (from within the Options dialog).
Close FF.
Reopen FF.
Check your cookies.

If you again have cookies similar to those in your screenshot, then I'd think you've got some general malware on your computer.

If the same sort of cookies do not show up, then possible an extension you are using in your original Profile is doing some work "behind the scenes"?

I created a clean profile and now I can delete the folders in cookies.
