Hi, I use Telegram (web and stand alone version) and sometimes some emojis do not show up
Instead I get a question mark or a code like this:
Any idea how to fix this?

p.s. I've already cleared fonts cache (but that didn't fix it)

Some emojis aren't supported on some operating systems. The developers want emojis to render more consistently with other applications on the same system. The developers care less about supporting all emojis.

I know I have an old OS.. I thought it was a simple fix but looks like it's not
I wish there was a way to put the code online somewhere and to see at what emoji belongs

I wouldn't update font files or install new fonts like Lotesdelere. That's kind of extreme.
The following prefs are equal to "Segoe UI Emoji, Twemoji Mozilla" and -1 on my system.
* font.name-list.emoji
* gfx.bundled-fonts.activate
I can set the font.name-list.emoji pref to "Twemoji Mozilla, Segoe UI Emoji" to change the font priority.
I can set the gfx.bundled-fonts.activate pref to 1 to always load bundled fonts.
Remember to restart the app after changing the prefs.

Note that on Mac, font.name-list.emoji might default to "Apple Color Emoji" and you may not have "Twemoji Mozilla" or not all new E14 and newer emojis might not be supported.

Twemoji Mozilla is a bundled font that is located in the "C:\Program Files\Mozilla Firefox\fonts" folder on Windows.
