The "Send Later" extension replaces the traditional "Send" button by one with two sub-options: "Send now" (the default) and "Send Later", which processes the message through the extension. Normally, when the extension is installed, clicking on the "Send" button sends the message immediately as if the extension wasn't installed.

HOW TO CHANGE THE BEHAVIOUR

In the "Send Later" options, the 1st tweak is "Replace 'Send' by 'Send Later'"
When you select this option, clicking on the button "Send" works as if you clicked on the submenu and select "Send Later": it brings a new large dialog box where you select either the date and time you send the message (ex.: Oct. 1st, 2:15 a.m.) or the delay before it is sent (ex.: in 15 minutes).
If you send the message that way, then it will be timestamped at the time the message is actually sent in the world.

Short answer, the extension doesn't change anything. So the time displayed on the recipient's computer is the time you clicked on "Send" and not the time it was actually sent to the world

Long answer: You can combine both for interesting effects.
Let's say you click "Send Later" (via the extension) at 15:00 and delay the sending for 2 hours, so Thunderbird's extension will send it at 17:00. But you are offline until 18:00. The recipient's computer will say that the message was sent at 17:00, i.e. at the time the "Send Later" extension released it.
