There are already quite a few (External Application Buttons, for instance, relies on Firefox's toolbar customisation to work). It's up to the authors. Ever since toolbar customisation was first checked in there have been plans to do it in a more "windowsy" way by creating a "toolbox" element which contains icons and can be moved around different toolbars / locked into place. Nothing's set in stone yet.

I've just installed Firefox 0.9RC into my main computer. It automatically loaded all my IE favourites, my home page (Google), and when I pressed F11, the toolbar is one line!

A big thank you to the developers. Firefox is becoming my default browser.

Alas, after adding two extensions, Adblock and Download With, and restarting Firefox, the title bar is back and I cannot get rid of it.
