I think the t-shirt offer is going out to people that have AMO-hosted add-ons that are marked compatible with the Firefox 4 betas.

The t-shirts are being promoted for add-on developers who get their addons compatible with 4.0:

Never written an addon, nor would I know where to even start, and I got the offer...

Extensions to be more accurate as Addons is not one thing.
(*.mozillaZine.org is not Mozilla!)
The Complaint Department at Mozilla (current complaints) (<It is not a real complaint site but for jokes/testing of bugzilla).
The Complaint department link is borked until unknown time.

I think there are separate T-Shirts for addon developers and beta testers as I have seen another nomination form to the one lithopsian linked to. The form I saw that was for t-shirts, but not for making your addons compatible (can't remember where I found it though).

I posted a few bugs (of wich some dupes) and I received the email.
Unfortunately I can't remember if I double-checked the address. Let's wait! I live in Italy, it may take some time to be shipped, maybe it'll arrive as a present for Christmas (2011)!

Edit: I have a problem: the "ZIP/Postal Code" was 20034 the day I submitted the form. Unfortunately it was changed to 20831 or something similar three days after. Do I have to re-write the form?
