I've tried 3 times - 45.1, 45.2, and now 45.3 to update from 38. Every time, upon updating I am unable to download any messages. Running Windows 10.
I would just say never mind, but the hourly reminders to update get annoying.

Moving to Thunderbird Support.
I'm running 45.3.0 under Win 10 and see zero issues so I doubt that it's the program itself.
What error messages, if any, are you seeing when trying to get mail?
What security software are you running on that system?

I get no errors, simply no response at all. I'm running Avira but previously when I have tried I think I was running Avast or AVS. I've switched antivirus a couple of times.

Clear the browser cache and the Windows Temp directory.
Shutdown any anti-virus software you are running.
Download a new copy of the full installer from http://www.mozillamessaging.com/en-US/thunderbird/
Delete the directory C:\Program Files\Mozilla Thunderbird
Run the installer.
Restart the anti-virus program.
Note that your saved messages, address books, settings, passwords, etc (if any) are stored in a different directory from the program files and will not be removed by deleting the directory identified above.

Try logging into webmail using a browser. Read and delete the oldest new message, log out of webmail and then try Thunderbird again. You may have run into either a captcha (which Thunderbird can't do anything about) or one of the messages was bad (causing a logjam as Thunderbird isn't smart enough to skip over it to the next one).

Still no luck, I tried both of these. Reverting to 38.x is the only way to read email.
