reply header is wrong

I've just noticed a weird situation. Whenever I hit reply, the header info consistently shows the wrong info. It shows the SAME wrong info each time. Somehow it picked up the info from an e-mail a few days ago, and now it keeps showing this no matter who I'm replying to, no matter what the subject. Thoughts???

I assume that you've installed my extension "Change quote and reply format" and that something is going wrong ... is it right?
If it's so, maybe you've an old version.
Go at my website and install the last one, the 0.4.9
