For your first screenshot: could you try to make the window a little bit wider?
For the lack of icon, the same case as abimtb extension, the developer points to a graphic that exists on the default theme but not necessarily on third-party themes. Look, I've expended a lot of time to make my themes work with Compact Header more or less nice. I could try to fix this writing extra code for the extension (again) as I patiently do until now. But I'm really annoyed, you know? If extension developers intend their extensions to work only with the default theme, if they don't care about third-party themes, why should I care about them? If it "works" with the default theme it's because they want explicitly that the extension work with the default theme. Why is this my fault?
It has been very difficult to me to make the theme for Thunderbird because I myself don't use Thunerbird, don't use Lightning, Compact Header MagicSLR and so on. If I do this is only because users... But, sincerely, I'm loosing the mood to further work on the Thunderbird theme.
